اهداف: ضعف ادراکی به عنوان بروز همزمان ضعف فیزیکی و اختلال ادراکی تعریف شده است. افسردگی دیرهنگام مرتبط با ضعف فیزیکی و نقصان ادراکی است، به ویژه سرعت پردازش و عملکرد اجرایی. هدف این مطالعه بررسی ارتباط بین ضعف فیزیکی و عملکرد ادراکی در افراد مسن افسرده است. طراحی: داده های اساسی شامل یک گروه افسرده در مطالعه هلندی افسردگی در افراد مسن است (NESDO). تنظیمات: مراقبت های اولیه و مراقبت های ویژه روانشناسی. مشارکت کنندگان: مجموعا 378 بیمار (60 ساله) مبتلا به افسردگی بر اساس معیار DSM-IV و نمره MMSE به میزان 24 یا بالاتر. معیارها: فنوتیپ ضعف فیزیکی و همچنین معیارهای آن (کاهش وزن، ضعف، خستگی، آهستگی، فعالیت کم).
عملکرد ادارکی در چهار حوزه بررسی شد: حافظه کلامی، حافظه کاری، کنترل تداخل و سرعت پردازش. نتایج: 378 بيمار افسرده (در محدوده سني 60 تا 90 سال، 1/66 درصد زنان)، 61 نفر قوی (بدون معیار ضعف)، 214 نفر پیش ضعیف (1 یا 2 معیار ضعف) و 103 نفر ضعیف (3 معیار) بودند. تحلیل رگرسیون خطی تنظیم شده برای این افراد نشان داد که شدت ضعف فیزیکی مرتبط با حافظه کلامی ضعیف تر (ß ¼ 0.13, P ¼ .039)، سرعت پردازش کمتر (ß ¼ 0.20, P ¼ .001) و حافظه کاری کاهش یافته (ß ¼ 0.18, P ¼ .004) بوده ولی ارتباطی با تغییر در کنترل تداخل ندارد (ß ¼ 0.04, P ¼ .54). نتیجه گیری: در افسردگی دیرهنگام، ضعف فیزیکی مرتبط با عملکرد ادراکی ضعیف تر است، اگرچه در رابطه با عملکرد اجرایی همیشه این گونه نیست. مطالعات آتی باید بررسی کنند که آیا نقصان ادراکی در حضور ضعف فیزیکی به ضعف ادراکی تعلق دارد یا نه و همچنین باید زیرگروه خاصی از بیماران مسن افسرده شناسایی شود که به استراتژی های درمان چند حالتی شامل عملکردهای فیزیکی، ادراکی و روانی نیاز دارند.
Abstract
Objectives: Cognitive frailty has recently been defined as the co-occurrence of physical frailty and cognitive impairment. Late-life depression is associated with both physical frailty and cognitive impairment, especially processing speed and executive functioning. The objective of this study was to investigate the association between physical frailty and cognitive functioning in depressed older persons.
Design: Baseline data of a depressed cohort, participating in the Netherlands Study of Depression in Older persons (NESDO).
Setting: Primary care and specialized mental health care.
Participants: A total of 378 patients (60 years) with depression according to DSM-IV criteria and a MMSE score of 24 points or higher.
Measurements: The physical frailty phenotype as well as its individual criteria (weight loss, weakness, exhaustion, slowness, low activity). Cognitive functioning was examined in 4 domains: verbal memory, working memory, interference control, and processing speed.
Results: Of the 378 depressed patients (range 60e90 years; 66.1% women), 61 were classified as robust (no frailty criteria present), 214 as prefrail (1 or 2 frailty criteria present), and 103 as frail (3 criteria). Linear regression analyses, adjusted for confounders, showed that the severity of physical frailty was associated with poorer verbal memory (ß ¼ 0.13, P ¼ .039), slower processing speed (ß ¼ 0.20, P ¼ .001), and decreased working memory (ß ¼ 0.18, P ¼ .004), but not with changes in interference control (ß ¼ 0.04, P ¼ .54).
Conclusion: In late-life depression, physical frailty is associated with poorer cognitive functioning, although not consistently for executive functioning. Future studies should examine whether cognitive impairment in the presence of physical frailty belongs to cognitive frailty and is indeed an important concept to identify a specific subgroup of depressed older patients, who need multimodal treatment strategies integrating physical, cognitive, and psychological functioning.
